The Balkan Nephropathy
نویسنده
چکیده
This book attempts the awesome task of compiling a fairly complete bibliography of the clinical and experimental publications through 1964 on phenothiazines, butyrophenones, meprobamate "like" agents, and MAO inhibitors. Approximately two thirds of this book is bibliography with the remaining one third a narrative which is little more than an anotated bibliography. Both sections forsake critical evaluation for comprehensiveness. There is no attempt to emphasize the most pertinent studies or clarify contradictory results and consequently the reader is left with little more than a mass of seemingly random facts. The chapter on "The Biochemical Basis of Mental Disease" exemplifies this; it is almost an appendage with little effort to correlate this data with the preceding sections on the psychopharmacologic agents themselves. The studies noted in this book demonstrate the gap between the sophistication of the laboratory studies on psychopharmacologic agents and the naivete of some of the clinical studies (not to mention the low correlation between efficacy in the laboratory setting and in actual clinical trials). The very mass of data presented in this volume (2,000 studies on chlorpromazine alone) demonstrates the importance of communicating this information effectively to the clinician. A recent study showed that most physicians' drug administration habits are influenced in order of frequency by (1) the example of teachers and peers, (2) testimonials from colleagues, (3) extrapolations from specific studies, (4) convenience, (5) persuasiveness of advertising and manufacturer's representatives, and (6) availability of liberal samples. Undeniably, the present volume fulfills its function as a source book, but in view of the brevity and uncritical nature of the presentation, the reader will still have to consult the original sources to evaluate the findings cited. Hopefully, future volumes will attempt to evaluate and synthesize the data in this important area.

متن کاملRole of exposure analysis in solving the mystery of Balkan endemic nephropathy.
We evaluated the role of exposure analysis in assessing whether ochratoxin A or aristolochic acid are the agents responsible for causing Balkan endemic nephropathy. We constructed a framework for exposure analysis using the lessons learned from the study of endemic goiter within the context of an accepted general model. We used this framework to develop an exposure analysis model for Balkan end...
متن کاملTubuloepithelial - Myofibroblast Transdifferentiation - Possible Pathogenic Mechanism of Interstitial Fibrosis in Balkan Endemic Nephropathy
Balkan endemic nephropathy (BEN) is a chronic tubulointerstitial disease of unknown etiology. The main morphological feature is interstitial fibrosis and tubular atrophy with absence of inflammatory infiltration. The pathogenesis of BEN is also obscure. Since tubular epithelial cells in the early phase of disease express vimentin as mesenchymal marker in addition to cytokeratin, we could specul...
متن کاملEnvironment, Medical Geology and the Etiology of Balkan Endemic Nephropathy
Background Balkan endemic nephropathy (BEN) is an irreversible, chronic, tubulo-interstitial nephropathy of unknown origin, geographically confined to several rural regions of the Balkan Peninsula. The first "official" observation of a disease resembling BEN was made almost six decades ago in regions comprising the former Yugoslavia (Danilovic et al., 1957). Shortly thereafter, similar descript...
متن کاملWeathered Coal Deposits and Balkan Endemic Nephropathy
The correlation between the geographic occurrence of Balkan endemic nephropathy (BEN) and low rank Pliocene lignites is striking. Comparison of mass spectra of methanol extracts from lignite samples collected in the vicinity of two endemic villages, with mass spectra of methanol extracts higher grade coals, shows the presence of many more potentially nephrotoxic compounds, and much higher total...
متن کاملNature of the Virus Associated with Endemic Balkan Nephropathy
ndemic Balkan nephropathy (EBN), a disease restricted to three Balkan countries (Bulgaria, Rumania, and Yugoslavia), is characterized by a progressive shrinking of the kidneys and, in some cases, tumors in the proximal regions of the urinary tract (1,2). A coronavirus was reported to be involved in the etiology of the disease, mostly on the basis of the isolation of a virus in cultures of kidne...
